Chernobyl: The Forty-Second Flaw

On April 26th, 1986, at 1:23 in the morning, an operator at Chernobyl Nuclear Power Plant pressed the emergency shutdown button. It is designed to stop the reactor. The reactor exploded forty seconds later.

The Soviet investigation blamed the operators. The IAEA's revised report, published five years later, did not.

This is the case file — a primary source reconstruction of what the documents actually show: the design flaw, the prior knowledge, and the five-point revision that changed the official account.
